<h3>Select your behavior goal statements</h3>
<p>You have the option of selecting and editing pre-defined goal statements for your behavior report card by clicking on any of the behavioral categories below. Or you can create your own custom goal by clicking on the 'Add Custom Goal Statement' link.</p>

<h3>Generate the report card</h3>
<p>You can generate your Behavior Report Card in several formats: Daily, Daily by Time of Day, Daily by Location, and Weekly. You can also click the 'Preview' button at any time to view any one of these report card types. </p>

<p>You can insert up to 5 different time periods in the 'Daily by Time of Day' version of the Behavior Report Card. To select a time span, (1) click on the Start Time box and enter a start time, (2) click on the End Time box and enter an end time, and then (3) click on the Add Time of Day link to save that time-span. </p>

<p>You can insert up to 5 different locations in the 'Daily by Location' version of the Behavior Report Card. To select a location, (1) type a location into the text box and then (2) click on the Add Location link to save that location. </p>

<h3>Enter the basic form information</h3>
<p>Behavior Report Cards are customized behavior rating forms that educators can use to evaluate targeted student behaviors on a daily basis or even more frequently. </p><br>
<p>Use this application to create your own Behavior Report Cards in pdf format with rating items unique to the students that you are rating. You are welcome to create Behavior Report Cards at no cost for use in your school or home setting and to share them with others. </p><br>

<p>Complete the fields below as the first step in creating your Behavior Report Card.</p>

<p>You may save this customized goal statement to use in future report cards. Select a category under which the goal is to be saved and then click the Save Goal Statement button.</p>

Entering a name is optional. However, if you enter a student FIRST name, that name will be incorporated into every behavior goal statement to personalize the Behavior Report Card. If you enter a student LAST name, that name will not appear in the Behavior Report Card.

You can type in any title for your Behavior Report Card.

You can edit the instructions below or write your own. The instructions will appear on the final Behavior Report Card that you create.

Select the font to be used in creating the Behavior Report Card.

Select the size of the characters in the text of the Behavior Report Card.

If you endorse this checkbox, a signature block will appear at the end of the report card to be signed by whomever you specify (e.g., parent, principal).

You can edit the instructions for the report card signer or write your own instructions. These will appear above the signature block in the final Behavior Report Card.

If you type the name(s) of the person(s) responsible for filling out the rating form, the name(s) will be displayed in the final Behavior Report Card. The default entry is 'Teacher' unless you change it.

You can specify a classroom or other information about a student's program. If you type an entry into this box, it will be displayed in the final Behavior Report Card.

If you select a gender for the student (and also enter the student's first name in the previous box), the program will automatically rewrite all behavior goal statements in the Behavior Report Card to incorporate student name and gender-appropriate pronouns.

You can specify who will sign the Behavior Report Card (e.g., parent, principal, mentor, coach).

You can edit this custom goal statement before saving.

Select a category under which to save your custom goal. If no existing category appears appropriate, place the custom goal in the 'Other' category.
